What to expect from the DOT-SAP Process:
I do not work for the government, DOT, your employer, or any treatment facility.
I am a licensed therapist, a certified drug and alcohol counselor, and DOT-certified SAP.
My role is to evaluate your needs for education or treatment, monitor your progress, and provide written recommendations to your employer (or designated employee representative (DER) at the start and end of the SAP process.
The initial DOT-SAP evaluation:
Clinical interview in a face-to-face evaluation session
Recommendations for education or treatment
Referral to an education or treatment provider
Written summary of recommendations to the DER
The follow-up DOT-SAP evaluation:
Face-to-face follow-up session
Recommendations for any further education or treatment
Recommendations about eligibility to return to work (at employer’s discretion)
Written summary of recommendations to the DER
Your responsibilities include:
Participate honestly in the initial DOT-SAP evaluation
Schedule an intake with the recommended provider within 2 business days of the evaluation
Attend the intake within 10 business days of the evaluation
Successfully complete the initial recommendations
Remain completely clean and sober
Do not work in a safety-sensitive position until you are cleared to return to work
Provide random drug and/or alcohol tests as required
Schedule a follow-up evaluation with me upon completion of the initial recommendations
Participate honestly in the follow-up DOT-SAP evaluation
Participate in any aftercare recommendations

What’s an SAP and why would you need one?
The SAP, or Substance Abuse Professional, is a specially qualified substance abuse clinician who plays a central role in the Return to Duty process of an employee who has tested positive for drugs or alcohol.
The SAP is neither an advocate for the employer nor the employee.
The SAP’s overarching function is to protect the public interest of safety by professionally performing specific DOT-mandated Return to Duty functions.
As an employee, when you have violated DOT drug and alcohol regulations, you cannot again perform safety-sensitive duties for any employer until and unless you complete the SAP evaluation, referral, and education/treatment process set forth in the subpart and in applicable DOT agency regulations. The first step in this process is a SAP evaluation.
The SAP process
After you have been referred as a consequence (positive test) of engaging in substance use related conduct according to the Department of Transportation (DOTS) Rules and Regulations, the following will occur:
(1) You have been notified by a Medical Review Office (MRO) that your test results are positive for drugs or the Breathalyzer Alcohol Technician (BAT) notified you of a positive alcohol test.
(2) DOT mandates that you be referred to a SAP for an evaluation in resolving problems associated with misuse of alcohol or the use of controlled substances.
(3) When your SAP recommends treatment, you and your employer will be given a written recommendation specifying the approximate duration of treatment and the level of care.
(4) The SAP will provide you with an appropriate referral which is consistent with clinically evaluated needs and will take into consideration your insurance coverage, your ability to pay for care, and employer’s policies regarding availability of a leave of absence.
(5) If you have been terminated from employment and are pursuing the evaluation in hopes of obtaining another safety sensitive position, the SAP’s recommendation may be provided to you and a copy retained for any future employer.
(6) As a DOT client, you are expected to participate in treatment as outlined by the treatment provider and the SAP. If you resist or fail to comply, the treatment provider will discharge you for non-compliance. This course of action would prevent you from working in a safety sensitive position.
(7) The SAP will hold a follow-up evaluation to review your compliance with the initial treatment recommendation.
You will be required to follow the treatment provider’s recommendation for continuing care and meeting attendance.
(8) The SAP will submit in writing to your employer a summary outlining the initial assessment and participation in treatment. The SAP will also recommend to the employer that you pass a Return to Duty test for drugs or alcohol, plus order the mandated DOT follow-up testing schedule.
